**[ESSENTIAL FUNCTIONS [1. Follow established performance standards and perform duties according to nursing service policies and [procedures. 2. [Perform resident care techniques and procedures according to certification instruction, in-service education, [or instruction from supervisor. 3. [Identify resident problems and concerns and report them immediately to a charge nurse or a licensed nurse. 4. [Conduct resident rounds as assigned. 5. [Initiate corrective action as necessary and/or seek assistance of a licensed nurse. 6. [Identify safety hazards and emergency situations, and initiate corrective action. 7. [Assume personal responsibility for following facility procedures related to control of equipment and supplies [within the neighborhood. 8. [Document incidents and/or unusual problems according to established facility procedures. 9. [Participate in the development of an individualized plan of care of assigned residents. 10. [Review care plans and perform nursing care as outlined. 11. [Observe and verbally report to the Charge Nurse accurate and timely information concerning assigned [residents, such as change in medical condition or health status, lack of response or resistance to care, [special problems, unusual incidents, resident needs or requests, behavioral or psychosocial concerns; [document in writing according to established procedures or as requested. 12. [Comply with procedures for safe lifting, transferring, and handling of residents; assume responsibility for [safety of residents under immediate supervision or in caseload assignment at any given time; report and [document incidents/injuries according to established procedures. 13.
