POSITION OVERVIEW

Responsible for coordinating with the behavioral health team and other community resources to support and optimize patient health outcomes.

SCHEDULE AND PAY

Clinic Open: Monday – Thursday 8AM - 7PM and Friday 8AM – 5PM. Scheduling needs will be discussed during interview. The pay depends on experience and start at $34-36/hour.

DUTIES

  • Responsible for managing behavioral health EHR tasks.

  • Manages voicemails and completes follow up within 2 business days.

  • Manages and completes prior authorizations within 2 business days.

  • Follows up on staff messages and in-basket tasks within 2 business days.

  • Deliver quality, comprehensive nursing care to behavioral health patients.

  • Assists in rooming patients as necessary

  • Triages patients in person and via phone as necessary

  • Dispenses medication and coordinates medication refills

  • Facilitates medication administration on long-acting injectable antipsychotics

  • Secures medications to dispense from Safe Net RX

  • Starts IV therapies as necessary

  • Provides wound care as necessary

  • Provides patients with information about their condition, treatment options, and self-management strategies.

  • Documents all patient interactions, education, and outcomes within 24 business hours

  • Manages schedule for all substance use disorder patients (monthly drug screenings)

  • Provides care coordination services and evidence-based interventions.

  • Facilitates transportation of patients to hospital when necessary

  • Supports medication management through treatment adherence education

  • Supports providers through providing additional education to patients as needed

  • Assists patients with medication assistance paperwork

  • Completes prior authorizations as needed within 24-48 business hours

  • Provides follow-up to patient care as requested/needed.

  • Facilitates integration of care within ACHC departments and outside entities.

  • Assists other departments in opening patient’s appointment access to providers

  • Coordinates referrals with Access Services team for patients in need of community resources

  • Assists in coordinating health services with patients referred to ACHC

  • Maintains established relationships with community partners

  • Participates in initiatives with community partners to improve access to services

  • Other duties as assigned.

QUALIFICATIONS

  • Registered Nurse(RN) required

  • Bachelor’s degree in Nursing from an accredited nursing program preferred

  • One year of full-time work-related experience in agency and/or medical clinic service delivery (counseling, case management, outreach, prevention/education, etc.)
