Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.

Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown.

As a Clinical Services Manager, you will be responsible for assigned programs that integrate services focused on clinical services for target populations. Ensures compliance with assigned grant and regulatory requirements and achievement of grant objectives. Develops and maintains community partner and funder relationships. Responsible supervision and leadership for assigned staff. Responsible for providing treatment adherence interventions and ensures compliance with program and/or grant requirements and patient care management through quality assurance measures.

    What You Will Do

  • Manages assigned programs including supervision of Nurse Care Managers, Clinical Support RN and Program Support Worker, ensuring compliance with program guidelines. Establishes and communicates clear expectations, manages the provision of appropriate training and resources, and hold staff to account by monitoring work and providing feedback as needed.

  • Manages and participates in the development of assigned staff by providing coaching and guidance. Develops and maintains job descriptions, interviewing and selection of candidates, new employee onboarding , performance evaluations, competency assessments and ongoing training.

  • Ensures program grants are managed through participation in developing program budget, monitoring grant revenue and expenses, and making financially responsible decisions. Monitors monthly operations statements, approving daily expenditures to remain within established budget.

  • Executes program goals and ensures alignment with organizational strategic plan. Manages performance improvement activities for assigned programs including improvement initiatives, client and staff satisfaction, and safety activities. Participates in the development of assigned grant applications and renewals and other applicable grant requirements. Trains all staff on use and application of all software systems and holds staff accountable for accuracy and timeliness of staff documentation.

  • Develops and maintains community partner and funder relationships through proactive communications and participation in applicable workgroups and meetings. Facilitates referrals and educates others on PHC services as appropriate. Provides HIV education to clients/families/partners, internal staff, and external agencies in an organized and appropriate manner.

  • Conducts quality assurance measures of documentation and data fidelity, ensuring compliance with grant and regulatory. Monitors compliance with PHC policies and procedures, consistent with accreditation, regulatory, federal, and state rules and regulations.

  • Develops and facilitates departmental performance improvement initiatives for effective and efficient workflows, conducting quality assurance measures to ensure compliance with program guidelines.

  • Facilitates assigned programs including treatment adherence interventions, care management and maintenance of EMR documentation. Assists staff in developing effective care-related treatment plans. Assists clients in accessing support services by serving as a liaison with other community agencies. Facilitates financial assistance processes as needed to include the Ryan White slide, AIDS Drug Assistance Program and the pharmaceutical Patient Assistance Programs.

    Qualifications You Need to Bring

    Required:

  • Bachelor’s Degree in Nursing or related field or equivalent combination of education and experience.

  • A minimum of 3 years of experience in nursing, public health, or related field.

  • Knowledge of nursing care in accordance with regulations and practices established by the Iowa Board of Nursing.

  • Team oriented with ability to work collaboratively and build/maintain professional relationships at all levels.

  • Effective verbal and written communication skills.

  • Proficiency using Microsoft Office applications and internet-based applications; willingness and ability to learn different software and databases.

  • Excellent problem-solving skills with ability to assess issues, identify root causes and formulate viable solutions.

  • Sound leadership skills with ability to engage, inspire, and guide people to successful outcomes.

  • Organization, prioritization and time management skills with ability to multi-task in a fast-paced environment.

  • Professional skills with ability to interact in a calm and empathetic manner.

    Preferred:

  • Experience in public health and grant funded programs.

  • Experience in HIV related care.

  • Work experience in a community health center.

  • Bilingual verbal and written proficiency in English and Spanish.

    Your experience and skills determine your base pay. The hiring range for this position is typically $63,600 - $79,500 annually. Candidates with extensive work experience related to this position may be considered for additional compensation up to the pay grade maximum of $95,400 annually.
