Easter Seals of Iowa Personal Service Coordinator in Des Moines, Iowa

Easterseals is seeking a compassionate and dedicated Personal Services Coordinator to join our team!

In this position in Easterseals AIM program, (Achieving Independent Methods), you will play a vital role in supporting individuals with disabilities to achieve independence and well-being. You will provide a variety of services to ensure clients' safety, health, and overall well-being are being preformed within the guidelines of Easterseals policies. You will work collaboratively with clients, families, and other professionals, to develop individualized plans and support clients in achieving their goals.

Principle Accountabilities:

  • Client Care: Oversee the daily personal health, safety, and well-being of each client, ensuring adherence to Easterseals and Polk County BHDS guidelines.

  • Skill Development: Train and assist clients with instrumental activities of daily living- including cleaning, personal hygiene, leisure skills, safety skills, cooking, budgeting, shopping, and housekeeping. Additionally, support clients with somatic care needs, such as medical appointments and following through with medical recommendations.

  • Housing and Employment: Assist clients in finding safe and independent housing as well as obtaining and maintaining employment opportunities.

  • Transportation: Train clients on using public transportation and arrange or directly transport them to appointments, work, or leisure activities.

  • Resource Coordination: Refer and coordinate additional funding options like Vocational Rehabilitation, HCBS Waivers, Medicaid, and Medicare.

  • Mental Health Support: Stay current on mental health issues and protocols, providing support to divert clients from jail or psychiatric hospitalization.

  • Collaboration: Build relationships and maintain communication with families, providers, and case management agencies.

  • Outcomes and Documentation: Track client progress towards goals, document activities, and complete required reports.

  • Performance Management: Monitor, evaluate, and track client outcomes, making recommendations for improvement.

  • Budget Management: Manage within a team budget and make recommendations for improvement.

  • Additional Duties: Perform other duties as assigned.

    Knowledge, Skills, and Experience:

  • Bachelor's degree preferred or one year of experience working with people with disabilities.

  • CPR and First Aid certified within 90 days

  • Med Manger certified within 90 days

  • Mandatory Reporter of Child and Dependent Adult Abuse trained within 30 days

  • Ability to carry an on-call cell phone (approximately one week every eight weeks).

  • Ability to obtain and maintain an Iowa driver's license (Class D as needed).

  • Insurable under Easterseals vehicle insurance policy.

  • Reliable transportation to transport clients.

  • Ability to work under pressure, maintain accuracy, and meet deadlines.

  • Excellent written and verbal communication skills.

  • Ability to work effectively with diverse populations.

  • Computer proficiency with Easterseals database, Word processing software (Word and Excel preferred).

  • Ability to transfer or move up to 80 pounds.

  • Ability to bend, stoop, twist, and lift.

  • Flexibility to work non-traditional hours, including weekends and evenings.

  • Access to a telephone for the on-call system.

  • Ability to track and maintain position certifications and agency required trainings.

    Wage for this position starts at $15.00 an hour. This position is offered at full time position and typically Monday-Friday, although there might be the occasional situation that arises, (like needing to meet with a client in the evenings, weekend activities), with flex time available. There is an on-call rotation with this position that consists of approximately one week every eight weeks.     Interview process consists of 1st phone interview with Recruitment and Talent Development Coordinator and 2nd Department Specific, Team Leader interview.
