GENERAL SUMMARY: Provides patient care using physical agents and therapeutic exercise upon referral by and under guidance of a qualified physician. Work is performed with considerable independence within policy and program guidelines. Provides on-going physical therapy to patients of diverse ages and their families.

ESSENTIAL FUNCTIONS:

· Demonstrates professional, appropriate, effective and tactful written, verbal, and nonverbal communication with patient, families, medical staff, colleagues, vendors, and other departments throughout the continuum of care to promote continuity of care and services and enhance clinic image.

· Demonstrates positive professional customer service in all patient, staff, and visitor contacts

· Utilizes interpersonal skills to establish and maintain healthy interpersonal relationships with healthcare members, patients, and families.

· Acknowledges patient’s rights on confidentiality issues, maintains patient confidentiality at all times, and follows HIPAA guidelines and regulations.

· Participates in staff development activities and orientation as requested.

· Proactively acts as patient advocate, responding to and working to resolve patient concerns.

· CPR certified.

· Responsible for following equipment maintenance protocol, identifying problems, and coordinating appropriate repairs.

· Operation of various office machines (i.e. fax, copier, phone, and computer).

· Participates effectively as a team member in the clinic. Licensed Physical Therapist:

· Performs evaluations and treatments to a diverse population in a variety of settings (based on assignment) and completes supporting documentation according to clinic policy.

· Appropriately establishes and reviews: plan of care, goals and priorities with team (including patient/family), treatment in response to changing patient needs, educational needs of patient/caregivers, and discharge plan/follow-up care needs.

· Complies with department, clinic, hospital, and regulatory agency guidelines with respect to ethics, legal issues and supervisory relationships. Demonstrates professional accountability per the APTA Code of Ethics and Standards of Practice.

· Demonstrates appropriate and effective written, verbal, and non-verbal communication with team members through the continuum of care.

· Communicates with referring physician regarding patient treatment plan, progress and outcome of therapy.

· Demonstrates current level of knowledge of various payor regulations by functioning within those guidelines.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

· Certificate, Bachelor’s or Master’s degree in physical therapy.

· In-depth knowledge of anatomy and physiology as it relates to a variety of conditions.

· Licensed or eligible for licensure in Iowa.

· Previous work experience not required.

· Knowledge of current physical therapy principles and approvals.

· Obtain CPR certification within three (6) months of hire date
