ABOUT NACDD
The National Association of Chronic Disease Directors (NACDD) is a member-based Association that improves the public's health by strengthening state-based leadership and expertise for chronic disease prevention and management. NACDD also promotes social justice and well-being so communities can build healthier futures. NACDD’s core membership of the 59 State and Territorial Health Department Chronic Disease Directors and their staff, who protect the public's health through primary and secondary prevention efforts and work “upstream” on root causes of chronic conditions. In addition, NACDD unites 7,000 chronic disease professionals across the United States working in state, tribal, and territorial health departments, nonprofits, academia, and the private industry to promote health and reduce the burden of chronic disease. As a national, nonprofit professional Association, we advocate, educate, and provide technical assistance to inform programming and grow chronic disease prevention knowledge, leadership, and capacity.
PROJECT OVERVIEW
Expanding the Non-Communicable Disease (NCD) Collaborative in the Islands
This project supports US territories and freely associated states to address the increasing burden of type 2 diabetes by implementing strategies to improve the quality of diabetes care and increase access to diabetes self-management education and support (DSMES), the awareness of prediabetes and enroll eligible adults in the National Diabetes Prevention Program (National DPP) lifestyle change program. The Non-Communicable Disease (NCD) Collaborative aims to improve health outcomes by transforming clinical practice by integrating DSMES and expanding its focus to prediabetes and the National DPP. NACDD and the Association of State and Territorial Health Officials (ASTHO) will work collaboratively with CDC’s Division of Diabetes Translation (DDT) to work on the NCD Collaborative through various technical assistance (TA), and training modalities to facilitate skill building, program development, implementation, peer-to-peer learning, and partnership engagement using a data-driven planning process.
ROLE OVERVIEW
The consultant will serve as a diabetes subject matter expert and provide leadership, guidance, strategic direction, and project management, including budget and contract management, in tandem with NACDD’s Diabetes Portfolio Director and Diabetes Portfolio Manager. Further, this consultant will identify opportunities for synergy between other CDC-funded diabetes projects and provide expertise for quality improvement and peer learning.
SCOPE OF WORK
In close collaboration with the Diabetes Portfolio, the consultant will:
· Provide overall project management and coordination.
· Identify and engage partners with cultural, local context, and subject matter expertise to support implementing type 2 diabetes management and prevention activities.
· Establish NCD Collaborative Steering Committees.
· Lead and develop two regional five-year strategic plans to expand the NCD Collaborative's activities in the U.S. Affiliated Pacific Islands (USAPI) and the Caribbean (one plan for each region) to lead work and monitor progress responsive to regional strengths, trends, opportunities, and needs. The strategic plan will address supporting healthcare providers to conduct more systematic prediabetes screening and facilitate referrals.
· Create dissemination plans for the five-year strategic plans, working with internal (NACDD Communications, Pacific Chronic Disease Council, etc.) and external (CDC, ASTHO, and partner organizations) partners.
· Develop and implement technical assistance plans based on the five-year strategic plans.
· Facilitate the development of culturally and/or linguistically adapted programs or training materials needed to implement NCD Collaborative activities.
· Attend project team meetings with ASTHO, CDC DDT, and other identified partners.
· Ensure collaboration within the Diabetes Portfolio, NACDD’s Center for Public Health Leadership (CPHL), and other internal and external partners.
· Collaborate with other NACDD Portfolios and NACDD Councils and author/share successes/ideas, etc.
· Collaborate with the Diabetes Technical Assistance for Island Territories and Freely Associated States project.
· Complete required monthly programmatic reports and monthly invoices.

Requirements
EDUCATION
• Master’s degree in public health, public health administration, or related field.
REQUIRED CONSULTANT CREDENTIALS
· Experience in diabetes, health system quality improvement, or other chronic disease states, risk factors, and/or health equity.
· Experience with diabetes self-management and support and the National DPP lifestyle change program
· Experience working with State Health Departments and/or Island Territories and/or Freely Associated States (preferred).
· Experience working within USAPI, Puerto Rico, and or US Virgin Islands.
· Experience convening diverse groups of partners and facilitating meetings with focused agendas for public health action.
· Ability to work across time zones of many diverse partners.
· Demonstrated ability to build relationships and strong project management skills.
· Strong interpersonal and communication skills to build relationships with diverse partners.
· Strong strategic planning skills.
· Strong understanding of learning approaches.

ADDITIONAL QUALIFICATIONS
This position is an independent contractor position. To qualify to be a NACDD contractor, the applicant must have the following by the start of the contract:
• Establishment of a Limited Liability Corporation (LLC).
• Proof of insurance (general liability or professional liability).
• DUNS # (accessed through https://www.dnb.com/duns-number/get-a-duns.html).
• The Contractor shall comply with all guidance, including guidance conveyed through Frequently Asked Questions, as amended during the performance of the contract, for contractor or subcontractor workplace locations published by the Safer Federal Workforce Task Force (Task Force Guidance).
CONTRACT DURATION
Project Contract: 2/1/2025-7/31/2025): The contract could be extended, contingent on funding, with the expectation that funding will be multi-year.

Remote working environment. Not specific; ability to travel, up to five multi-day trips, including partner meetings, conferences, and other related meetings.
