Primary Health Care Registered Nurse Care Coordinator - RN - BEC in Des Moines, Iowa

Join our dynamic and collaborative team as a Nurse Care Manager where you'll have the opportunity to maximize your clinical skills across various specialties, including pediatrics, family medicine, walk-in care, and our Medication-Assisted Treatment (MAT) program. In this role, you will play a crucial part in assisting providers with the management of patients with chronic issues such as hypertension, diabetes, and medication management. Your day-to-day schedule ranges from seeing patients on your own schedule, supporting rooming staff as needed, and/or collaborating with the other two RNs to support clinic needs.

    What You Will Do

  • Supports and empowers clients throughout health care continuum to include holistic counseling, problem solving “barriers to care” issues and coaching clients as needed re: treatment and medication adherence, healthy life choices, and self-help behaviors.

  • Responsible for providing appropriate care and follow up to high-risk patients.

  • Facilitates RN-led patient visits.

  • Use monthly reports to monitor patient outcomes, and take appropriate follow up action with patients.

  • Fulfills Care Management Protocol.

  • Performs nursing assessment/medical triage of acute medical issues and medication side effects and refers or monitors appropriately.

  • Provides chronic disease and preventive screening education to clients/families/partners, internal staff and external agencies in an organized and appropriate manner.

  • Works with Care Coordinators to assist clients in accessing support services by serving as a liaison with other community agencies and facilitate financial assistance and entitlement applications processes as needed.

  • Manages patient referrals and follow-up to other specialty medical, mental health, substance abuse and community services.

  • Prepares/assists with monthly Quality Improvement program activities and facilitates development and ongoing activities for consumer involvement.

  • Serves as a patient advocate, resource, and point-of-contact for patients through the continuum of care. Provides care management of patients defined by the health center as high risk and/or needing education.

    Qualifications You Need to Bring

  • Completion of an accredited RN program accepted by the Iowa Board of Nursing for licensure

  • License & Certifications:

  • Current RN license recognized by the State of Iowa Board of Nursing

  • Current Basic Life Support for Health Care Providers Certification*

  • Current Mandatory Reporter Certification approved for the State of Iowa*

    *Must be obtained within Introductory Period if not current

    Your experience and skills determine your base pay. The hiring range for this position is typically $51,600 - $64,500 annually. Candidates with extensive work experience related to this position may be considered for additional compensation up to the pay grade maximum of $77,400 annually.
