Job Description:

This position functions as a vital member of the healthcare team who contributes to the success of the practice by demonstrating customer/patient focus, effective communication, professionalism, and teamwork. Utilizes the nursing process to assess,

plan, implement, and evaluate comprehensive care provided to selected patient

populations and their families.

This position is responsible for timely and effective management of patient calls requiring medical triage and coordinating care during the transition of care following an inpatient medical stay. Provides guidance, education and facilitates resources for the management of the at-risk population.

This is a virtual position, serving a region of the health system.

  • Shift Details: Full time (40 budgeted hours). Fully remote position. The schedule is five 8-hour days Monday through Friday. No weekends, No holidays.

  • Additional Details: This position will be responsible for inbound triage for primary care and post hospital transitional care calls. Must live in Montana with easy access to one of our care sites (near Butte, Bozeman, Billings, Miles City).

  • Preferred Qualifications: Acute care experience required and triage experience is preferred.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Performs clinical triage, including a comprehensive assessment, disposition and education using established triage protocols and guidelines

  2. Manages transitions of care to ensure appropriate follow up care and mitigate risks associated with readmission.

  3. Contacts patients following emergency department visits to ensure appropriate follow up

  4. Uses registry data to identify gaps in services and initiates intervention

  5. Refers patients to a variety of resources, including behavioral health, transportation services, nutrition, etc.

  6. Educates patients and families about health status, health maintenance, and management of acute and chronic conditions.

  7. Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations.

  8. Documents patient assessment and intervention data using established medical record forms/automated systems and documentation practices.

  9. Participates in multidisciplinary teams to improve patient care processes and outcomes.

  10. Interacts with all staff, colleagues and team members in a professional and collegial manner

Minimum Qualifications:

Required:

  • Graduate of an accredited school of nursing

  • Current/valid Registered Nurse license from the state licensing board is required.Basic Life Support (BLS) or Advanced Care Life Support (ACLS) is required.

  • One (1) year related experience in a clinic or acute hospital nursing setting is required.

Preferred:

  • Bachelor of Science in Nursing is preferred

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$34.19 - $50.73
