This individual works closely with physical, occupational, and speech therapists and assistants, utilizing treatment modalities, techniques and equipment to provide patient care as directed. Analysis and adaptive thinking required in the course of providing treatment. Provides on-going services to infant, child, adolescent, adult and geriatric patients and their families based on assignment; service provision is in accordance with discipline specific licensure laws or is provided as a function of a restorative plan that has been developed by a licensed physical, occupational, and speech therapist.

ESSENTIAL FUNCTIONS:

· Under indirect supervision of licensed therapy staff provides restorative treatment plans designed to maintain patient level of functioning in place of/ or is adjunct to other skilled rehabilitation services; carries out, monitors and/or adjusts restorative treatments utilizing therapy techniques, modalities, and equipment for which competency has been demonstrated; assumes responsibility for patient’s care, safety and environment throughout treatment; observes, reports and records all pertinent patient information according to department policy.

· Prepares patient, equipment, supplies (including topical medications used in burn/wound care; does NOT apply medicated supply to patient), and medical record for transport; verifies Rehabilitation Services order in chart; selects technique for transfer and equipment for transport according to established policies and procedures and therapist guidance; assumes responsibility of safe, timely transport of Rehabilitation Services patients and medical records to and from department clinics.

· Works in a collaborative effort with others: answers department telephone, taking complete message or answering questions related to patient schedule; assists in filing and record keeping; monitors patient during exercise.

· Assists in general maintenance of department: Cleans and stores equipment, restocks linens and supplies, changes linens and cleans treatment areas, maintains transport equipment and arranges for necessary repairs, empties soiled linen/waste containers

· Demonstrates appropriate and effective written, verbal and non-verbal communication with patients and their families, colleagues, medical staff, and the public; participates in staff meetings and inservice education; protects patient confidentiality in regard to medical, emotional, and social status.

· Retrieves relevant information from medical records; uses mainframe computer to maintain accurate scheduling, treatment and charge records; assists in general maintenance of work environment and equipment.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

· High school diploma or GED, required and 18 years of age.

· Basic Life Support (BLS) for the Healthcare Provider certified or obtained within three (3) months of hire.

· Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire
