Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.

Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown.

As a Street Outreach Worker, you will be is responsible for conducting street outreach in the community to jail, meal sites, camps, and non-traditional settings for unsheltered clients. Provides quality customer service, ensures accurate client data, and coordinates with team members to provide referrals to health care, emergency shelters, Centralized Intake and the Project of PHC. Completes testing for HIV/AIDS and uses principles of harm reduction to identify vulnerable clients for PHC-Project medical care and services. Demonstrates iCare values in daily work.

    What You Will Do

  • Conducts weekly outreach to non-traditional settings such as camps, congregate meal sites, and jail to ensure that unsheltered persons are connected to necessary services.

  • Completes HIV/STI testing in non-traditional settings such as shelters, camps, and congregate meal sites.

  • Provides referrals to client based on client eligibility for healthcare and/or mainstream resources.

  • Answers and responds to client contact, including phone calls, in a timely manner and records accurate and complete notes in HIMS according to PHC policies and procedures.

  • As appropriate, ensures that clients have access to transportation to necessary appointments. Assists with client transportation according to PHC transportation policy.

    Qualifications You Need to Bring

    Required:

  • Bachelor’s degree in related field or equivalent combination of education and experience (if allowed by granting agency).

  • Two year’s of work experience in a social work setting.

  • Excellent customer service skills.

  • Effective written and verbal communication skills.

  • Team oriented with ability to work collaboratively.

  • Basic computer skills with proficiency using Microsoft Office applications.

  • Detail orientated with high degree of accuracy.

  • Organization and prioritization skills.

  • Establishes and maintains effective working relationships with a diverse group of individuals.

  • Demonstrates professionalism with ability to maintain composure in all situations and show empathy.

  • Licenses & Certifications: certified as a mandatory reporter for child and dependent adult abuse or ability to become certified within 30 days of hire; active valid Chauffer’s Driver’s License

    Preferred:

  • Bachelor’s degree in health care, psychology, counseling, mental health, human services, or related field. Experience in characteristic duties of position may be substituted for advanced education if allowed by granting agency.

  • Demonstrated competency using HIMS.

  • Working knowledge of community resources related to homelessness in Des Moines.

  • Knowledge of program terminology.

    Your experience and skills determine your base pay. The hiring range for this position is typically $17.00 - $21.25 per hour. Candidates with extensive work experience related to this position may be considered for additional compensation up to the pay grade maximum of $25.50 per hour.
