Trinity Health Oncology Nurse Navigator - Cancer Center Services - Full Time - Days in Dubuque, Iowa

Description:

Customers include Patients identified within a specific target population, their families/caregivers, interdisciplinary care team, providers, acute care facilities, and post-acute facilities.

ESSENTIAL FUNCTIONS:

Adheres to the principles of caring and expected behaviors outlined in MercyOne Dubuque Medical Center's Model of Caring and Trinity Health's Code of Conduct, fostering a climate where all customers are treated in a manner consistent with MercyOne Dubuque Medical Center's Mission and Values. Maintains a working knowledge of applicable Federal, State and local laws and regulations, Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior. Follows patients throughout the continuum of care, including inpatient admissions, facilitating communication about the patient's plan of care between the patient, family/caregivers and all providers. Educates patient and families about the disease process, symptom management, treatment options and potential side effects. Accompanies patients to provider visits, when needed, and reviews information with patient afterward to ensure understanding. Assesses and identifies psychosocial needs and facilitates interventions aimed at maximizing coping and quality of life. Connects patients and caregivers with relevant internal and external support services. Discusses advanced care planning and assists with documentation of the patient's wishes and goals of care. Assists in decision-making processes, supporting patients and families in making decisions based on all available information and their own goals and preferences. Monitors the patient's treatment and follow up progression. Establishes partnerships within the interdisciplinary teams in order to align efforts with patient satisfaction outcomes. Educates patients about survivorship, set expectations for the post-treatment transition and remain available for questions. Works with hospital and cancer center administration to define quality metrics and strategic goals. Utilizes real-time data and generated reports to track key performance indicators and identify areas of concern. Works with the multidisciplinary team to meet or exceed the requirements relevant to navigation regarding communication, education, tumor conferences, research, community health needs assessment and quality improvement as identified by the Commission on Cancer. Participates in cancer conferences (i.e., tumor board) and assists with preparation of cases for presentation. Performs other duties consistent with purpose of job as directed.

Possesses knowledge of budget processes, quality improvement, report distribution, TJC standards, state and federal regulations. Demonstrates knowledge of clinical guidelines; Ensures compliance with current evidence-based guidelines, e.g., NCCN.

PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS:

Normal office environment where there are relatively few physical discomforts due to dust, dirt and noise. During an 8-hour day, an associate must be able to stand/walk for 3 hours and sit 5 hours. Job requires occasional (<10% of work time) squatting, crawling, crouching, kneeling, and balancing; frequent (21-50% of work time) bending, stooping, reaching above shoulder level, pushing and pulling. Job requires the ability to frequently (21-50% of work time) lift and carry objects weighing approximately 11-24 pounds. Job requires exposure to marked changes in temperature and humidity and may be exposed to infectious disease, hazardous waste and chemicals. Must have fine motor and manual dexterity skills to grip and manipulate items and perform repetitive action such as computer entry. Requires visual acuity, the ability to speak and the ability to hear speech. Job requires high level of mental concentration to attain established deadlines. Working environments includes frequent interruptions. Must be able to travel on occasion to other worksites and training seminars. Must adhere to applicable organizational requirements on an annual basis for Employee Health Directives.

MINIMUM QUALIFICATIONS & CORE COMPETENCIES

(EDUCATION, LICENSURE, CERTIFICATION, AND EXPERIENCE REQUIRED):

Education, Licensure and Certification Must be a registered nurse and must hold an active license to practice nursing in the State of Iowa. Bachelor of Science in Nursing required; Master of Science in Nursing preferred. Oncology experience preferred. Oncology Certified Nurse preferred. Child and Dependent Adult Abuse Mandatory Reporter Training is required within 6 months of hire and every 3 years thereafter. Basic Life Support Certification (BLS) will be required within 30 days of hire and every 2 years thereafter.

Experience:

  • Minimum of three years of clinical experience required.

Special Skills, Competencies, and Aptitudes:

  • Computer Proficiency

  • Required to evaluate care effectiveness (care process, outcomes and cost) for individual users of health care and patient populations.

  • Demonstrated experience and competency with Microsoft Office Suite, e-mail and a variety of web-based programs and browsers, with the willingness and ability to update and increase skills as needed to meet the goals of the departments.

  • Requires ability to use other software to perform the essential functions of the job.
