As a Case Manager, you will provide case management services to children and young adults with disabilities.

When You'll Work

  • Monday-Friday

  • Typical business hours

  • Occasional evening, weekend, or on-call responsibilities

    Benefits

    This role is full-time and eligible for health benefits including medical/dental/vision insurance, paid time off, and a 403B retirement account. PTO is frontloaded upon hire and starts accruing immediately. As a non profit organization, we participate in the Public Service Loan Forgiveness Program.

    What You'll Do

  • Coordinate the intake and eligibility process.

  • Develop, revise, and update the care plan, complete social history and produce documentation that is accurate and inclusive of all required information.

  • Provide education, resources, and support to families.

  • Coordinates and monitors services, maintains monthly contact with members of the team for each child and at least quarterly fact to face visit within the home or community.

  • Provides crisis intervention, mediates conflicts related to services.

  • Advocates on the behalf of the client and their families.

  • Maintain knowledge of current available resources, regulatory guidelines , policies and procedures, and data entry systems required for case management services.

  • Maintain the highest standards of professional conduct.

    What You'll Need

  • Required: Bachelor’s Degree in Human Services or similar. An Iowa License to practice as a registered nurse can substitute for degree.

  • Required: At least 30 hours in behavioral sciences, education, health care, human service administration, or social sciences is required within the bachelor’s degree.

  • Required: One year working with children or young adults with intellectual or developmental disabilities.

  • Knowledge of DHS and Polk County Health Services procedures, HCBS, ICF/MR regulations, and the social work process.

  • Good knowledge of community resources for families of children and young adults with disabilities.

  • Must be willing to travel statewide with occasional overnights.

  • Ability to communicate in English effectively for understanding, in writing and verbally.
