As a Case Manager, you will provide case management services to children and young adults with disabilities.

Benefits
This role is full-time and eligible for health benefits including medical/dental/vision insurance, paid time off, and a 403B retirement account. PTO is frontloaded upon hire and starts accruing immediately. As a non profit organization, we participate in the Public Service Loan Forgiveness Program.
What You'll Do
Coordinate the intake and eligibility process.
Develop, revise, and update the care plan, complete social history and produce documentation that is accurate and inclusive of all required information.
Provide education, resources, and support to families.
Coordinates and monitors services, maintains monthly contact with members of the team for each child and at least quarterly fact to face visit within the home or community.
Provides crisis intervention, mediates conflicts related to services.
Advocates on the behalf of the client and their families.
Maintain knowledge of current available resources, regulatory guidelines , policies and procedures, and data entry systems required for case management services.
Maintain the highest standards of professional conduct.
What You'll Need
Required: Bachelor’s Degree in Human Services or similar. An Iowa License to practice as a registered nurse can substitute for degree.
Required: At least 30 hours in behavioral sciences, education, health care, human service administration, or social sciences is required within the bachelor’s degree.
Required: One year working with children or young adults with intellectual or developmental disabilities.
Knowledge of DHS and Polk County Health Services procedures, HCBS, ICF/MR regulations, and the social work process.
Good knowledge of community resources for families of children and young adults with disabilities.
Must be willing to travel statewide with occasional overnights.
Ability to communicate in English effectively for understanding, in writing and verbally.
