Description
As the Director of Care Access and Experience, you will provide organizational oversight of an effective access and care coordination system across all programs and services. Your leadership will ensure exceptional care experiences across the organization, driving impactful outcomes for those we serve.
What You'll Do:
Provide organizational leadership and support to Case Management, Access Manager, and Social Workers. Lead, develop, and oversee organizational access and customer experience systems expertise across the organization.
Promote family partnership in a positive, collaborative manner and oversee and support all aspects of the family experience.
In collaboration with Care Council, work to improve the customer experience across the organization.
Identify and implement organizational standards for care coordination.
Promote organizational compliance and accreditation (e.g., Commission on Accreditation of Rehabilitation Facilities) to support organizational performance excellence.
Foster teamwork and strong communication and coordination and serve as a key liaison across organizational service areas and with external partners.
Participate in developing and aligning the strategic plan and identify organizational and departmental operating plan priorities.
Assure financial performance for service/department through frequent monitoring and corrective action when results are below projections. Collaborate with the accounting team on billing processes to meet reimbursement requirements.
What You'll Need:
Bachelor’s Degree in Social Work from an accredited school of social work. Master’s Degree in Social Work preferred.
Ten years of social work experience, including five years working with children/adults with special healthcare needs.
Five years of supervisory/management experience.
Ability to understand and communicate effectively and professionally in reading, writing, and speaking the English language.
