Role Overview
The Hospice Nurse Practitioner (NP) provides primary care under the supervision of the Hospice Medical Director, conducting face-to-face assessments for hospice patients in various settings. The NP collaborates with the Interdisciplinary Team (IDT) to determine eligibility, manage pain and symptoms, and ensure care aligns with professional standards and regulations. The NP may also liaison with referral sources and provide consultation.

10-20 hrs a week– typically 5-10 hours a week unless we get really busy then maybe 10-20
Supervision
Oversees RNs, LPNs, and Hospice Aides functionally
Essential Responsibilities
Conduct patient visits in homes or hospital settings. Conducts visits in patient homes, Skilled nursing and Assisted living Facilities, or hospital settings or virtual visits.
Assess and document patient eligibility for hospice services.
Collaborate with IDT, Medical Director, and Attending Physicians on care plans.
Prescribe medications and order diagnostic tests within scope of practice.
Educate patients/caregivers on hospice benefits and responsibilities.
Participate in IDT meetings and adjust care plans as needed. May attend virtually if needed.
Provide mentoring on hospice interventions and eligibility.
Qualifications
Master’s degree in Nursing; Nurse Practitioner certification/licensure.
DEA License (as applicable); active medical staff membership at Trinity Hospital.
Minimum 5 years of nursing experience (oncology, hospice, geriatrics preferred).
Strong communication, interpersonal, and grief management skills.
Valid driver’s license and reliable transportation.
Physical & Mental Requirements
Frequently lift, stand, and walk; occasionally position and lift patients.
Adapt to frequent interruptions and changing work parameters.
Travel to various work locations in all weather conditions.
