MercyOne Sioux City is seeking a compassionate and skilled Home Health Licensed Practical Nurse (LPN) to join our dedicated healthcare team. The LPN will provide in-home nursing care to patients, supporting their medical needs and promoting their independence. This role involves working closely with patients, families, and interdisciplinary healthcare teams to deliver high-quality, personalized care in the comfort of patients' homes.

Key Responsibilities:

  • Provide direct patient care under the supervision of a Registered Nurse (RN) or physician, including medication administration, wound care, and monitoring of vital signs.

  • Collaborate with healthcare providers to develop and implement individualized care plans based on patient needs and physician recommendations.

  • Educate patients and their families on disease management, medication administration, and lifestyle modifications to improve overall health outcomes.

  • Perform routine assessments and document patient conditions, treatments, and progress in accordance with healthcare regulations and MercyOne policies.

  • Coordinate with the interdisciplinary team to ensure comprehensive, patient-centered care is delivered.

  • Monitor and report any changes in patients' health status to the supervising RN or physician promptly.

  • Maintain up-to-date knowledge of home health care protocols, infection control practices, and safety guidelines.

  • Ensure proper use of medical equipment, supplies, and documentation software.

  • Comply with regulatory and MercyOne standards for home health care, including participation in quality assurance and performance improvement programs.

  • Assist in coordinating services such as physical therapy, occupational therapy, and social work as needed.

Qualifications:

  • Active Licensed Practical Nurse (LPN) license in the state of Iowa.

  • Minimum of 1 year of clinical nursing experience, preferably in home health or community-based care.

  • CPR and Basic Life Support (BLS) certification required.

  • Strong interpersonal skills and the ability to work independently in a home setting.

  • Knowledge of home health regulations and documentation practices.

  • Proficient with electronic health records (EHR) systems and basic computer skills.

  • Valid driver’s license and reliable transportation, with willingness to travel to patient homes in the Sioux City area.

Preferred Qualifications:

  • Experience with wound care, medication management, and chronic disease management.

  • Familiarity with home health equipment and telehealth technology.

  • Experience with geriatric or hospice care.

Physical Requirements:

  • Ability to lift, carry, and transfer patients as needed.

  • Ability to drive between home visits.

  • Must be able to walk, sit, and stand for extended periods during patient visits.
