WesleyLife Home Health provides a full range of home health care services across central Iowa, serving clients in Boone, Dallas, Hamilton, Jasper, Marion, Marshall, Polk, Story, Warren, Cass, Washington, Audubon, Guthrie, Adair, and Poweshiek counties. A Day in the Life as a Nurse Case Manager with our Home Health team:
Lead and supervise the home health team by managing schedules, approving time off, overseeing productivity, and providing feedback for performance reviews.
Train, coach, and mentor new and current team members, ensuring they are up to date on processes and electronic medical records (EMR) systems.
Review charts weekly and organize care conferences with the team to ensure quality care.
Travel within the service area to assess clients, create personalized care plans, and make adjustments as needed.
Coordinate with physicians to ensure clients receive the appropriate care and services, while communicating any changes in their condition.
Manage discharge planning and coordinate with other services to ensure a smooth transition.
Participate in on-call duties for evenings, weekends, and holidays as required.
Address and resolve complaints and ensure compliance with policies.
Open Shifts Available: Full time, Mon - Fri with rotating on call weeknight or weekends

C ompetitive Pa y: The pay range for this position is $70,031-91,000/year. The starting pay rate will be based upon the years of experience.

Active LPN or RN License in the state of IOWA
Ability to pass a pre-hire background check, pre-employment health assessment and drug screen.
Valid driver's license, reliable transportation, valid insurance, and clean driving record
Preferred
- One year of home care experience is preferred.
