Description:

The LPN provides one on one care to a disabled client who requires care for 10hours a day M-F while her parents work. Medication administration, feeding, toileting and skin care, ROM, cognitive stimulation. Additionally the LPN would support the field RNs by making home visits for general assessments, medication set up, wound care, other needed cares, patient/caregiver education, coordination of care with the care team.

Position Purpose:

Provide 1:1 care for a disabled client in their own home, 2 days a week. Schedule will include in home visits to other patients. Cares include medication administration, feeding, toileting, skin care and cognitive stimulation.

What you will Do:

Care for clients in their own home.

Document and report cares.

Support RNs with other patients as needed.

Hours/Schedule:

.9 full time, Monday through Friday

10 hour days

Minimum Qualifications:

Active IA LPN license

LPN or Pediatric experience preferred.
