Great River Health Systems Social Worker | 40 hours per week | Home Health and Hospice in West Burlington, Iowa

Minimum Hiring Wage:

From $26.89 per hour

Scheduled: 8:00am to 5:00pm Monday - Friday

Job Description:

Addresses the social, emotional and financial needs of patients and their families as these needs relate to their health care needs and concerns, including transition planning. Demonstrates knowledge of process of death, dying and bereavement. Demonstrates the ability to work collaboratively as a member of the Home Health/Hospice Interdisciplinary Team. Demonstrates understanding of the psychosocial factors operative in terminally ill patients and their families. Assists in training of volunteers and staff regarding psychosocial needs of the dying patient/family and bereavement as requested.

Qualifications:

Required

  • Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education; or

  • Baccalaureate degree in Social Work (BSW) from an institution accredited by the Council on Social Work Education; or

  • Baccalaureate degree in psychology, sociology, or other fields related to social work, and is supervised by an MSW; and

  • One year of social work experience in a healthcare setting
