TELLIGEN HCBS Specialist in WEST DES MOINES, Iowa

The HCBS Specialist works directly with participants and HCBS professionals to positively affect the quality of life of thousands of people who are at risk for institutionalization because of a physical or mental health condition or developmental or cognitive disability. This HCBS Specialist position has a focus on HCBS member experiences and will evaluate the quality of member experiences related to person-centered planning, the use of self-directed services, experiences as participants in HCBS waiver and Money Follows the Person programs, and more. The HCBS Member Experience Specialist will work with case managers and providers to remediate issues found through the evaluation process and provide technical assistance and support throughout the quality oversight review processes.

Essential Functions

  • Assesses, documents, and reports on gaps and/or promising practices in provider performance and recommends improvement methods and processes to assist providers in delivering high-quality support and services to members.
  • Assists in the implementation of the methods and processes.
  • Partners with team members and internal/external customers as necessary, to help determine, evaluate methods, and mentor to encourage participation in the quality initiatives. This may include research of interventions, data analysis, formal and informal education, or presentations.
  • Develops and maintains activity and/or project plans.
  • Ensures plans, tasks, and timelines meet contractual and Company requirements and guidelines.
  • Provides regular updates to Company management on activity and/or project status.
  • Researches and gathers technical and clinical data to support activities and/or project initiatives.
  • Performs analysis on the results of the data collection. Based on the outcome of the analysis, determines areas for improvement.
  • Provides recommendations and feedback to project participants.
  • Assists in the development of project materials and intervention tools.

Requirements

  • Experience as a clinical professional, community health professional, public health professional, community development professional, or social work professional
  • 3 - 5 years experience working in direct collaboration with providers, community organizing, population health or public health, and/or healthcare quality improvement in specific areas as needed and designated by the contract or project.
  • Organization and time management skills
  • Critical thinking skills
  • Customer service skills
  • Knowledge of Medicaid and HCBS
  • Case management experience preferred
  • Professional writing skills
  • Functional PC knowledge

Skill/Certification Preferences

  • Knowledge of the following healthcare quality principles (preferred):
  • Medicaid services, organizations, and programs
  • Information management processes, electronic health records, and healthcare information technology
  • Quality improvement concepts, principles, and process tools
  • Basic statistics, data collection techniques, data analysis, and graphic representation of data
  • Effective communication both written and verbal
  • Public speaking
  • Policy and procedure development
  • Training development and presentation skills
  • Community Organizing
