TELLIGEN Nurse Care Manager in WEST DES MOINES, Iowa
As a Nurse Care Manager, you will be responsible for the care management of patients with acute and chronic medical conditions and the assessment of care needs of vulnerable populations for various clients.
Work Hours:
8am to 12:30pm CST 3x week , 3pm to 7pm CST 2x week, there is flexibility around other commitments.
Hybrid Schedule: 2 days per week in office
Essential Functions
- Performs assessment and evaluation of potential care management patients per client contractual obligations and/or state requirements.
- Collaborates with health care providers and ancillary service providers to validate the appropriate treatment plan and identify educational opportunities.
- Assessment/education may include face-to-face interaction for specific contracts.
- Provides ongoing education.
- Communicate with all health care providers to ensure appropriateness of care.
- May recommend and make adjustments in care if needed.
- Evaluate treatment adherence and reevaluate the patient's commitment to make the necessary changes.
- Documentation per contractual requirements related to individual member status, and/or medical necessity review, and negotiation.
Requirements
- Four-year degree in health care or related field and/or equivalent training and/or experience
- Current RN license in Iowa
- Minimum 1 year experience in human services and/or 3 years clinical practice experience
- Self-directed, assertive, great critical thinking skills
- Experience with Motivational Interviewing preferred
- Certified Diabetic Educator preferred
